PYWC SKIN CONDITION FORM
Any wrestler who has a skin issue that has not been reported will be sent home until it
can be verified that they have had the proper treatment and a form is submitted.
MISSOURI USA WRESTLING SKIN CONDITION FORM:
If a wrestler has any skin condition and has been released to wrestle by a physician,
you can print this form and have the physician fill out and sign it. Please have this form with you
in hand when weighing in/skin checks. At most tournaments, you will not wrestle if a skin
condition is found and you do not have this form.
Tournament Registration Guidelines:
⚠️ Once a registration is submitted on Track Wrestling, there are no refunds.⚠️
If you need to cancel a registration, you must contact Laura Kelley or Randi Rose before the club's registration deadline.
Selecting Weights:
Most tournaments will use a rounded weight from a drop-down list. Your wrestler must weigh in at or below the selected weight on weigh-in day. Each event may handle weight entry slightly different. Follow the instructions on the form for that week.
Multiple Wrestlers or Double Bracketing:
If you are registering more than one child or entering a wrestler in multiple divisions, please complete a seperate form for each wrestler and/or division. After you submit the first one, simply reload the page to start a new form.
Double Check Your Info:
We will post a preliminary registration list on Facebook and TeamSnap the Thursday before the club's deadline and again on Sunday for final review before closing registration. Please check your child's name, division and weight carefully and submit a new form for any corrections needed or report errors to Laura immediately.
Updating Information:
The details you submit in this form are what we use to register your wrestler. If anything changes, such as weight, age group, or spelling errors, please submit a new form. The most recent submission will always replace the older one.
Payment and Completion:
Registrations are only considered complete when:
- The form is filled out completely.
- There are enough funds in your wrestler's account to cover the entry fee.
Disclaimer: Some tournaments hosted through online platforms charge additional fees at the time of registration. PYWC is NOT able to determine in advance which tournaments include the fees. If a tournament does charge additional online registration fees they will be automatically deducted from your account when registering your wrester.
